GENERAL ORDER TO BE ISSUED TO ALL BANKRUPTCY PRACTITIONERS PROVIDING NOTICE OF NEW AND AMENDED OFFICIAL FORMS APPROVED BY THE JUDICIAL CONFERENCE; PROMULGATING REVISIONS TO NEW OFFICIAL FORMS AS MADE NECESSARY BY LOCAL RULES OF PRACTICE; AND INVITING COMMENT FROM THE BAR WITH RESPECT TO SUCH REVISIONS

RONALD G. PEARSON, Judge

Please take notice that the Judicial Conference at its September 1997 meeting approved two (2) new Official Bankruptcy Forms and amendments to nine (9) current Official Forms.

The new forms are Official Form 20A Notice of Motion and Official Form 20B Notice of Objection to Claim. The amended forms are listed below.

Official Form 1 — Petition pages

Official Form 3 — Application and Order to Pay Filing Fee in Installments
Official Form 6F — Schedule F — Creditors Holding Unsecured Nonpriority Claims
Official Form 8 — Individual Debtor's Statement of Intention

Official Form 10 — Proof of Claim Form

Official Form 14 — Ballot for Accepting or Rejecting a Plan
Official Form 17 — Notice of Appeal under 28 U.S.C. § 158(a) or (b) from a Judgment, Order or a Decree of a Bankruptcy Judge

Please note that use of the new and amended Forms is mandatory March 1, 1998. Whereas the format of the Official Forms proscribed provided sufficient latitude for this Court to adapt the new forms to comply with existing local rules of practice, notice is hereby given that this Court proposes revision of new Official Forms 20A and 20B, as shown in the forms attached to this Order. Form 20A is intended to be substituted in place of Local Form F-2. The Court wishes to extend to counsel the right to object or comment on the revisions made to these Official Forms to facilitate practice before this Court. Accordingly, it is

ORDERED GENERALLY that the revised Official Forms 20A and 20B be distributed to all practitioners who make regular appearances before this Court and that the Court will consider objections and comments to such revisions during the ensuing thirty (30) days.

Objections, comments and requests for copies of the new forms should be sent to the Clerk, United States Bankruptcy Court, Southern District of West Virginia, Post Office Box 3924, Charleston, West Virginia, 25339-3924, 304/347-5114.
